Form 19 & 10C- Withdrawal of Provident Fund and Pension Fund (The EPF & MP Act, 1952) - The Employees Provident Funds and Miscellaneous Provisions Act, 1952

When you send these forms, the receiver will get an email form filling instructions, checklist,all relevant forms attached, list of desired documents and Government proofs.

Who should use this?
Employees who are either retiring or who are leaving their service

Which are the forms & what is their purpose?
(1) Form no. 19 is for Withdrawal of Provident Fund
(2) Form no. 10-C is for withdrawal of Pension Fund
How to fill the forms?
To ease your work, please read the instruction carefully. The instruction sheet will be sent to your email-id, along with the forms

Mandatory checks on the form
* 3 Signatures on Form no. 19
* 2 Signatures on Form no. 10-C

Documents to be submitted along with the forms
(1) Cancelled cheque along with bank statement/ first page of bank passbook
(2) Self-attested photocopy of your PAN card
(3) Form no. 15-G (Declaration from the member confirming that his total income during the year is not taxable)
(4) Form no. 15-H (For Senior Citizen 60 & above)

Strict checks on the bank account of the employee (beneficiary)
# The claimant should attach a copy of Bank Passbook first page/cancelled cheque ensuring that the IFSC code account number are clearly visible.
# should be a single account or joint account with spouse only

# As per circular dated 18-Aug 2015, revenue stamp is not required for withdrawals
# As per amendment w.e.f. 01-Jun 2016, TDS will be deducted if a person is a member of PF for less than 5 years and amount of withdrawal payable is more than INR 50,000/-.
